Checklist for the Coordinated Program in Dietetics
All checked items listed below are required for a complete application. If items are missing, your application may not be processed.
- All prerequisite courses completed by the end of spring semester of the year you wish to enter the program, with a minimum overall grade point average (GPA) of 2.75.0. Click here for prerequisites. Conditional admission may be offered to students who anticipate completing this coursework during the summer semester.
- All prerequisite courses completed with a grade of “C” or better.
- Complete the on-line Texas Common Application.
Submit the following materials and documents to the Registrar at the address above, before the application deadline:
- $45 application fee. Your application cannot be processed without payment being received by the deadline date.
- Official transcripts from every college or university you have attended – If official transcripts are sent to you, you may send them to the Registrar if the seal on the envelope is not broken.
- Two (2) reference forms – Click here to download or print reference forms.
- Personal Statement – addressing interests, motivation, career goals, commitment and knowledge of the profession.
- Request for Waiver of Program Prerequisites (if required) – Necessary if you wish to request substitution of a program prerequisite course a waiver of program prerequisites. Include a copy of the course number and title, description, college/university where it was taken, and state which prerequisite that it will substitute for. - TOEFL score (Test of English as a Foreign Language) – Required only of international applicants from countries where English is not the native language; minimum score of 68 (Internet), 220 (computer), or 560 (paper) is required.
- International Transcript Evaluation (international students, only) – Transcripts of coursework taken at colleges/universities outside the United States must be submitted to the Registrar, along with a course-by-course evaluation by an approved credentialing organization.
